Request Milk As A Hospital

If you are an established NYMB customer, please submit your order request with the purchase order to [email protected].

If you are not an established customer and do not have a tissue bank license for human milk, click here to apply to obtain a New York State milk bank license. In New York, donor milk is considered a tissue and hospitals in New York must be licensed by the New York State Department of Health. There is no fee for the license.

The New York Milk Bank offers Star-K Kosher certified pasteurized donor human milk. Please follow the process below to inquire about Kosher Milk availability or to learn more about our certification.

Steps for Using Donor Milk in a New York State NICU

Obtain support from Neonatology and Hospital Administration
- The New York Milk Bank (NYMB) provides a presentation to your hospital staff and administration
- Who should attend? Nurses, Lactation Specialists, Neonatologists, Pediatricians, Tissue Bank, and Purchasing & Billing
File an Application with NYSDOH
- If adding to an established Tissue Resources Program at the hospital: Form 2973(C)
- If establishing a new Tissue Resources Program for Donor Milk: Forms 2973 and 2973(C)
Add The New York Milk Bank as a Hospital Vendor
- Request vendor application from purchasing department and email to [email protected]
- Review and sign NYMB contract (if the hospital is not part of a contracted network with NYMB)
Create a Donor Milk Hospital Policy
- Establish criteria for receiving, storing, dispensing, and tracking PDHM
- Procedures for storing, handling, and dispensing PDHM are similar to those for mothers’ own milk
Designate Storage & Distribution Location
- PDHM can be stored on a separate shelf or compartment in the same freezer where mother’s own milk is stored or in a separate freezer
Establish a Milk Tracking System
- Track receipt and distribution of PDHM
Designate and Train Personnel
- Nurses or milk technicians
- PDHM is handled similarly to mother’s own milk
Order Milk
- Email your purchase order to [email protected]
Provide Consent Forms and Patient Education Materials
- Informed consent
- Upon request, NYMB will provide rack cards and informational handouts for recipient families
